New Client / Patient Registration

To complete your registration:

  1. Review the practice policies outlined here and the examination and travel fees here prior to submission.

  2. Fill out the form completely.

  3. Email a full copy of your pet's medical history to TheVillageVetMA@gmail.com.

    • Include all vaccination certificates, exam notes and lab results​

 

Things to note:​​

  • Due to the high volume of new patient requests, please allow 7-10 business days for your registration form and pet's medical history to be reviewed before we can schedule a visit.

  • New client wellness visits currently have a wait time of approximately 2-3 months.

For hospice and peaceful passing visits: please call (413) 224-8384 to leave a voice message to request these services

Submission of this form indicates that you agree practice policies and agree that payment is due in full at the time of services (please note that we are unable to accept Care Credit as a form of payment).
