New Client / Patient Registration

IMPORTANT ANNOUNCEMENT:

Due to an overwhelming amount of support and trust from our current clients, we have reached capacity for wellness patients. We are still accepting new registrations, but they will be placed on our waitlist and you will be contacted if an appointment becomes available. Please continue care with your current primary care veterinarian until that time.

**We will continue to maintain openings for in-home euthanasia visits, as helping pets pass in the peace and comfort of their home is one of our practice missions. Please call us directly at (413) 224-8384 to discuss scheduling these peaceful passing visits.**

REGISTRATION DIRECTIONS:

1. Review the practice policies outlined here and the examination and travel fees here prior to submission

2. Fill out the form completely.

3. Email a full copy of your pet's medical history to records@thevillagevetma.com.

Include all vaccination certificates, exam notes and test results
Appointments will not be scheduled without your pet's full previous history being received and reviewed

I understand that The Village Vet currently has a waitlist for new Wellness patients. Your application will be reviewed and kept on file. If an opening becomes available, you will be contacted via email.

I agree to the Village Vet's practice policies. Submission of this form indicates that I am the legal owner of this pet and over 18 years of age, able to make any and all medical decisions necessary for my pet. I understand that payment is due in full at the time of the visit and accounts sent to collections will be responsible for any associated charges
